What to do if you want to get Massage Therapy under your Insurance:*
1. Call the phone number on the back of your Insurance Card.
2. Ask if you have Massage Therapy coverage that is provided by a Licensed Massage Therapist (L.M.T.). Sometimes Insurances will cover Massage Therapy ONLY if it is provided by a Chiropractor, Doctor, OR Physical Therapist.
3. Ask what your co-pay will be (if any), this will be due at the time of service. If you are checking an Insurance that is NOT listed above, ask if they cover an OUT OF NETWORK L.M.T. provider. (Skip to Step #5)
4. Ask how many Massage visits you get in a year and if those are just Massage. Often your visits will come out of a "pool" of visits that is for all Complimentary Medicine including Massage, Acupuncture, Chiropractic, and Naturopathic Medicine.
5. Ask if a deductable needs to be met in order for insurance to pay. If yes, ask how much of your deductable has already been met.
6. Ask if you need Pre-Authorization or a prescription for Massage Therapy. Sometimes if you have a prescription you can pay with your Health Spending or Flux Spending Account.
7. For OUT OF NETWORK L.M.T. PROVIDER, ask your insurance company where you can submit a Superbill (Provided by Amy Beth Jacobsen L.M.T.). This will prove the treatment was paid for by you, so you can get reimbursed.
What to do if you are wanting to get treatment for a Worker's Compensation or Motor Vehicle Accident Claim (M.V.A.):*
1. Please get a Referral or Prescription from you Chiropractic/Medical Physician. Also, please make sure your physician puts the ICD-10 Diagnosis codes on the referral.
2. Your first visit will be longer than normal so please arrive 15 min early and expect to be in the office for at least 2 hours.
3. Please have your Claim Number available to put on your intake form as well as the name, address and phone number of your insurance and Claim Representative's name. I will also need to know the State that the incident took place.
M.V.A.: Please ask your Claim representative if your plan is a PIP (Personal Injury Protection) or Liability.
Worker's Compensation: Please list the name of your Lawyer if you have one.
4. Typically, in Acute Injury Recovery, Massage Therapy has been proven to offer great healing and medical benefit for a limited time. This DOES NOT mean that more Massage Therapy will not be needed, it only means the Insurance Company will only cover so much. Please ask your Claim Representative if there are any limitations to how many visits you can have.
